Appointments

Appointments in our office can be made over the phone or requested through the Patient Portal. If you are new to our office, we ask that you call to set up your first appointment. We will need the name of your child, date of birth, and insurance information. We make every effort to schedule your child with the primary care physician of your choice. However, we will sometimes offer appointments with a different clinician or a different location due to availability or specialty. Our scheduling staff should always confirm the clinician, location, date and time with you when scheduling. You can learn more about our clinicians on the staff page of our website. Completing a New Patient Form prior to your first visit will expedite the check-in process.

Well Child Check Ups

We see children of all ages for Well Child and Sports Physical appointments. We follow the American Academy of Pediatrics Guidelines for all well child check ups.

Sick Visits

If your child is sick, we do offer same day appointments as available. We do allow walk in visits, but we ask that you be patient as we will see pre-scheduled appointments prior to walk-in patients.

Behavioral Health

We are able to meet most behavioral health needs at our office. In the event that we are not able to meet a behavioral health need, we will provide referrals to other specialists.

The initial appointment needs to be scheduled with a provider for a complete physical. You will be given a packet of paperwork to be completed and returned within 45 days. Once the packet has been received, the second appointment is scheduled with a provider experienced in behavioral health issues.

Because insurance benefits vary in coverage, we strongly suggest that you contact your insurance carrier for specific behavioral health coverage.

Please complete a Behavioral Health Form prior to your visit.

Referrals

If you feel your child needs to be referred to a specialist, please call our office for an appointment. Your child will need to be seen by one of our providers before a referral can be made.

If your PCP is referring your child to a specialist, please allow 7-10 business days for our staff to complete the referral. Once the appointment has been scheduled with the specialist, our staff will call you with the appointment date, time, address, and phone number. Some specialists will contact you directly to schedule once the referral order has been received.

To avoid delays please be sure to provide our office staff with a current working phone number and an alternative phone number as well as any preference in location, clinic, days, or times for the appointment with the specialist. If you are unable to keep your appointment with the specialist, it will be your responsibility to contact the specialist's office and reschedule.
